GDPR - We use the personal information you provide to identify an appropriate class for you/your child and to ensure you/they are well supported and safe whilst participating in gymnastics. All personal information will be held securely and will only be shared with coaches or others who need this information to provide gymnastics activity and meet your/your child's needs. If you would like more information on how we use information about you/your child, please see our privacy notice or contact us. I agree for the information I have provided to be used for carrying out risk assessments and reasonable adjustments and understand that the club may contact me if they require any further information. 

I confirm that to the best of my knowledge, the named person is physically fit and healthy and I have declared any medical information that the club needs to consider prior to allowing them to participate in gymnastics activity. Please be warned that, although we will take every step to ensure the safety of the gymnasts, every sporting activity carries with it a small risk of injury. In such an event we cannot be held liable unless negligence is proven.

I consent to emergency medical treatment or first aid, which, in the opinion of a qualified medical practitioner or first aider is considered necessary. I also understand that should such a situation arise, all reasonable steps will be taken to contact the parent or an alternative emergency contact.

I consent to the named person being photographed/video footage whilst participating in club activities/events and for these images to be used for promotion, education and development of the club/sport e.g. to publicise events, in newspaper articles, magazines and other media such as websites, information leaflets, electronic newsletters and presentations. Once an article or image is published and in circulation (e.g. newsletters, website images) it can be copied and used by others. However, you can contact us at any point and ask us not to use your image or words for any new purposes. We will comply with this request and will also do our best to stop others using your image or words, although we cannot guarantee this. (Please note that we will be unable to remove images that have already been used in publications or publicity material). If you do not withdraw your consent, we will retain any images for three years. After three years, images will be reviewed and will be deleted unless they are considered to be of historical importance to the club and are retained as part of the club's archive.

British Gymnastics memberships to be completed within 2 lessons of joining or renewed every Sep/Oct, payable directly to them [british-gymnastics.org]
